Terms of Use for Member Database

Overview

These Terms of Use explain how the Arizona Coalition for Healthcare Emergency Response (AzCHER) handles information submitted to the AzCHER-Connect database.

These Terms of Use are incorporated into, and considered a part of, the Participation Agreement. By submitting a Participation Agreement or continuing your organization’s membership in AzCHER, you agree to most current version of these Terms of Use.

Our Database

When you submit a Participation Agreement, if your organization is approved as a member, then the information you provide about your organization will be stored in our database, AzCHER-Connect, which is accessible at https://connect.azcher.org. Information in AzCHER-Connect is visible to all members. Users must be members who have a username and password to log into AzCHER-Connect.

The information that will be visible to other members in AzCHER-Connect will include your organization’s name and contact information, as well as any individual contact information you provide (such as an individual’s mobile phone number).

Your Control Over Information

Changes to Profile in AzCHER-Connect. An organization may modify the information included in the organization’s profile in AzCHER-Connect as follows:

Deactivation of Profile in AzCHER-Connect. Any organization, whether a member of AzCHER or not, may request deactivation of the organization’s profile in AzCHER-Connect by sending an email to azcheradmin@azhha.org.

Our Right to Modify Information

AzCHER has the right to modify an organization’s profile in AzCHER-Connect at any time, in order to correct information that AzCHER believes is inaccurate or out of date. Where reasonable, we will attempt to contact an organization for confirmation of the correct information before modifying the profile in AzCHER-Connect. However, such confirmation is dependent on the accessibility of the organization’s designated representative or other personnel.

Opting Out of Email Communications

AzCHER members may opt out of email communications from AzCHER through their profiles, by unchecking the “Notifications” box located next to your contact details within your profile.

Non-members of AzCHER may opt out of email communications by emailing the administrator at azcheradmin@azhha.org.

Username and Password Security

A username and password is required to log into AzCHER-Connect and view the member listings. It is important to select a strong password, one that is different from the password you use for any other account. You should not share your username and password with anyone. Users should carefully protect their usernames and passwords.

No Guarantees

Although we strive to keep the information available in AzCHER-Connect current and accurate, errors can occur. We make no guarantees about the accuracy of any information in AzCHER-Connect. AzCHER will not be liable to you, or to any other person or entity, for any harm that arises from or relates to the use of AzCHER-Connect, or any information provided in that database.
